This order form is for RACE CAR DRIVESHAFTS only.

In order to manufacture your custom driveshaft correctly, we need accurate information and measurements.

PLEASE NOTE
If your information is incorrect, your driveshaft will not fit
and will require extra work, costing more money for labor and shipping.

Please Measure all dimensions accurately with vehicle in running position (weight on suspension). Fill in all dimensions requested on this form. All material and workmanship is guaranteed, except when used in four wheel drive vehicles and in racing applications. A $75.00 deposit is required to process your order. NO PERSONAL CHECKS ACCEPTED. Cashier's check or money order only for non-credit card purchases.

You will be contacted with an estimate before we begin work. Orders are shipped UPS Ground within seven working days of estimate. All shipments will be C.O.D.
